Activities
Case tracking is the anchor to collecting data for service delivery, CYAC activities within the core area of case review may include:
- MDT members actively involved with the case discuss updates, issues, and concerns with the goal of ensuring all partners have necessary information
- MDT members debrief following the forensic interview to share relevant information to determine next steps
- Urgent case reviews are held in situations where case changes may impact safety, justice, and healing for the child/youth and their family (e.g., custody disputes or changes, immediate safety concerns)

CYAC GuidelinesOutputs
The types of activities at your CYAC will determine what outputs you track related to case reviews. Example outputs are:
- # of unique case reviews
- # of repeated case reviews
- # of partners involved in case reviews
- Case review decisions
Child and youth outcomes
Case reviews within the CYAC model can contribute to safety, justice, and healing for children and youth. Ways to know how case review activities contribute to these outcomes are:
Safety
- Ongoing needs of the child/youth and family are met
- MDT members appropriately share information to facilitate efficient and effective service delivery
Justice
- Children, youth, and caregivers feel empowered to share their thoughts/concerns
- Children, youth, and caregivers are informed of recommendations or decisions from case review in a timely way
Healing
- Children, youth, and caregivers have access or are referred to the supports and services they need and a warm hand-off is provided (if service is outside CYAC)
Data Collection Practices
Data collection practices need to reflect your local operational and community context. You may already have tools in place to track these outputs and outcomes, or have tools that can be adapted to reflect these outputs and outcomes.
